Job summary

Prairie Community Services is seeking a Direct Support Provider for the Heartwood Home in Morris, MN. This full-time, on-site night shift role entails supervising, caring for, and ensuring the safety of individuals served, including daily living activities and community orientation.

The starting wage is $11.41 per hour plus a $2 shift differential. Benefits and a mission-driven team support the well-being of staff and clients alike.

Qualifications

• Minimum age 17 or as required by licensure.
• Must pass DHS background clearance.
• Must have valid driver's license.
• Ability to read, write, communicate and understand English.
• Follow each person served's program-related plans and procedures.
• Deliver support to the person served on principles of person-centered services.
• Administer CPR/First Aid when needed.
• Chart daily program information.

Responsibilities

• Provide supervision, care and safety to individuals served in daily living, recreation, and community activities.
• Assist with medication management, meal preparation, laundry and cleaning.
• Support individuals in following person-centered plans and procedures.
• Document daily progress and program information.
