Job summary

Sawtooth Mountain Clinic in Grand Marais, MN, is seeking a licensed mental health clinician to provide diagnostic assessments and non-pharmacological treatment in an integrated primary care setting. Collaborate with the Behavioral Health Director and a multidisciplinary team including MDs, NPs, RNs, and MAs.

Role combines teamwork and independent accountability in decision-making. The position requires LICSW/LMFT/LPCC licensure (or provisional) and a Master’s degree.

Qualifications

• Minnesota license as LICSW/LMFT/LPCC or provisional license.
• Master’s degree or higher in psychology, counseling, social work, or marriage and family therapy.
• Minimum of two years direct patient care; primary care setting preferred.
• Team care model experience and patient-centered care philosophy.
• Proficient in documenting clinical care and MS Office applications.
• Knowledge of assessment, planning, care coordination, and patient rights principles.

Responsibilities

• Assess and diagnose patients using specialized knowledge and assessment tools.
• Develop patient care/treatment plans based on clinical expertise and patient complexity.
• Provide outpatient therapy services as appropriate.
• Complete diagnostic assessments according to standards of care.
• Monitor patient outcomes and response to care; adjust care as needed.
• Document patient care in the clinical record per guidelines and HIPAA.
