Qualifications

• Master's degree in social work and two years of supervised therapy experience in behavioral health and/or medical social work including diagnostic assessment, individual, group, and family therapy.

• International degree evaluation must be completed by a credential evaluation service approved by NASBA/NACES.

Responsibilities

• Conduct diagnostic assessments and determine clinical interventions.
• Deliver short-term psychotherapy and patient/family education.
• Develop and implement treatment and transition plans.
• Refer patients to community resources to address barriers related to social determinants of health.
• Participate in huddles and case consultation meetings.
