Job summary

Signify Health, part of CVS Health®, is seeking a Full Time Physician to provide in-home health evaluations. You will visit members at their homes, perform clinical assessments and ensure appropriate connections to care. This role requires an active medical license and willingness to travel locally to licensed states.

Qualifications

• Active, unrestricted license(s) in coverage area(s).
• Multi-state licenses (preferred).

Responsibilities

• Provide in-home health evaluations for health plan members.
• Build trusted relationships to improve health outcomes.
• Engage with members during visits and perform clinical assessments.
• Travel locally and within licensed states as needed; conduct virtual visits as required.
