Job summary

Crossbridge Hospice in Youngstown, OH is seeking a compassionate Hospice Aide to join our care team. This full-time role provides personal care and supportive services to patients in their homes or residence, helping maintain dignity, comfort, and quality of life.

The ideal candidate is a certified nurse’s aide (CNA) or State Tested Nursing Assistant (STNA) for Ohio, with a valid driver’s license and reliable vehicle.

Qualifications

• Must be a certified nurse’s aide (CNA). State Tested Nursing Assistant (STNA) for Ohio.
• Must maintain a valid driver's license, good driving record and reliable automobile
• High school graduates or GED preferred
• Demonstrate the ability to be able to work within a multi-disciplinary team setting
• Demonstrate the ability to work in field situations

Responsibilities

• Provide compassionate personal care to patients in their place of residence.
• Assist with at least 2 of the following ADLs including personal hygiene.
• Assist patients with medication intake and prescribed exercises.
• Perform incidental household services essential to patient health care.
• Follow the Plan of Care and update as instructed during visits.
• Apply the 4Ms framework (What Matters, Medications, Mentation, Mobility) and provide Age-Friendly Care
