Job summary

AdvisaCare Hospice is seeking a compassionate Hospice RN in Michigan to provide one-on-one home-based care, guided by dignity and comfort. You will work with a team to tailor plans of care and support patients and families through the final days with presence and empathy.

This full-time role includes on-call rotation and flexible scheduling to accommodate family needs while delivering high-quality hospice care. Field visits and interdisciplinary collaboration are essential components of the job.

Qualifications

• Current Michigan license in good standing.
• Minimum 1 year hospice experience.
• Empathetic approach to patients and caregivers.
• Participate in field visits daily, weekly office meetings as indicated.

Responsibilities

WE'RE LOOKING FOR A FLEXIBLE RN!

30–40 hours per week. Includes rotation of the on-call schedule. A flexible opportunity for an RN who wants to work around family schedules while continuing to make a meaningful impact through nursing

As a Hospice RN with AdvisaCare Hospice, you’ll provide emotional support and compassionate clinical care, helping patients experience their final days with dignity, comfort, and respect.

You’ll also:

• Conduct appropriate supervisory visits
• Provide clinical observation and feedback to assigned team members
• Participate in interdisciplinary group meetings
• Help develop, review, and revise patient/family plans of care
• Advocate for the needs and wishes of patients and their families

• Initial and on-going assessment of patient’s physical, functional, psychosocial and environmental needs
• Implement the individualized plan of care and revisions as needed
• Compliance with AdvisaCare Hospice Policies and Procedures, Medicare compliance
• Consulting with and educating the patient/family on disease process, self-care techniques, end-of-life care, dealing with issues of ethical concern
• Supervision of paraprofessionals providing services to patient
• Preparing clinical and progress notes, participating in IDG conferences
• Participate in Hospice performance improvement program
• Maintain high level of customer satisfaction with our clients, families and caregivers
• Communication of information using current process and technology within organization

Requirements:

• Current Michigan License in good standing
• Minimum 1 year hospice experience
• Empathetic approach to patients and caregivers
• Participate in field visits daily, weekly office meetings as indicated
• Ability to adapt to patient’s status and needs on a full time on call basis
