Job summary

AdvisaCare Hospice is seeking a dedicated RN to provide in-home Hospice care in the Bay City/Midland/Saginaw Tri-City area. This full-time role focuses on compassionate, patient-centered management in the home setting.

Benefits include a supportive team, no on-call requirement due to after-hours coverage, and opportunities for advanced training. A Michigan RN license and hospice experience are required to join AdvisaCare Hospice.

Qualifications

• Current Michigan RN license in good standing.
• Minimum 1 year hospice experience.
• Empathetic approach to patients and caregivers.
• Participate in field visits daily as indicated.
• Ability to adapt to patient status on full-time on-call basis (if required).

Responsibilities

• Initial and ongoing assessment of patient needs (physical, functional, psychosocial, environment).
• Implement and revise the plan of care as needed.
• Ensure compliance with policies and Medicare requirements.
• Educate patient and family on disease, self-care, end-of-life care.
• Supervise paraprofessionals in patient services.
• Prepare clinical notes and participate in IDG conferences.
• Contribute to hospice performance improvement efforts.
• Maintain high levels of client and family satisfaction.
• Communicate information using current processes and technology.

Are you looking for a challenging and rewarding position as a Full time Hospice RN Case Manager? We are searching for compassionate, dynamic, energetic, organized and efficient professionals with Hospice experience to become part of our Hospice Family for our Bay City/Midland/Saginaw servicing the Tri City areas

For this role, you need to be a registered nurse who sees the value of Hospice care. You will be providing emotional support, with the goal to help patients live their final days with dignity and comfort. Our Hospice RN's will conduct appropriate supervisory visits which allow for clinical observation of performance of assigned team members. Participate and provide feedback to the team for regular interdisciplinary group meetings; on the development, review and revision of the patient/family plan of care.

• Maintain high level of customer satisfaction with our clients, families and caregivers
• Communication of information using current process and technology within organization

Requirements:

• Current Michigan License in good standing

• Ability to adapt to patient’s status and needs on a full time on call basis
