Job summary

AdvisaCare Home Health and Hospice is seeking a part-time/per diem MSW to join their hospice team in Grand Rapids, MI, serving surrounding areas with Troy as the parent agency. The role emphasizes compassionate care for patients with life-limiting illness and supporting families.

Ideal candidates have an MSW from CSWE-accredited programs, with a preference for those with home health or hospice experience and a valid driver’s license for community-based service delivery.

• MSW degree from a school of social work accredited by the CSWE.
• One year of experience in Home Health Care or Hospice preferred.
• Hospice experience preferred.
