Job summary

Your Health Organization is seeking a dedicated Certified Nursing Assistant (CNA) to service Hospice patients in the Carolina Forest-Forestbrook area. The role involves home and ALF visits, delivering medications and DME, and coordinating with the care team to ensure compassionate, patient-centered care.

Applicants must hold CNA certification, be willing to travel daily with reliable transportation, and work a full-time 8-hour shift (8:00AM-5:00PM, Mon-Fri).

Qualifications

• Certified Nursing Assistant certification in good standing.
• Phlebotomy experience preferred; training available if not already.
• Prior public health or home health experience preferred.
• Experience with DME delivery or logistics is a plus.
• Proficiency with technology tools for remote patient monitoring and EMR systems.

Responsibilities

• Work closely with the care team to ensure coordinated patient care per care plans.
• Communicate effectively by phone, email, Teams, and in person.
• Provide compassionate personal care to hospice patients at home, including bathing and mobility.
• Assist with safe mobility, transfers, and fall prevention.
• Deliver medications and DME, and reconcile medication lists.
