Job summary

Your Health in Conway, SC seeks a dedicated Certified Nursing Assistant to serve hospice patients in their homes and in ALFs. The CNA provides essential personal care, assists with ADLs, and helps coordinate visits with providers and pharmacy support.

This role involves travel daily, medication deliveries, and telehealth coordination, with 8-hour day shifts Monday through Friday. The team emphasizes compassionate care, safety, and timely access to equipment and medications.

Qualifications

• Certified Nursing Assistant license in good standing.
• Phlebotomy experience preferred; willing to train if not.
• Public health or home health experience preferred.
• Experience with DME delivery or logistics preferred.
• Proficient with technology for remote patient monitoring and EMRs.

Responsibilities

• Coordinate care with the care team to follow the care plan.
• Provide compassionate personal care for hospice patients at home and ALFs.
• Assist with mobility, transfers and fall-prevention.
• Deliver medications and review instructions; reconcile medications.
• Support telehealth visits and coordinate with pharmacists and providers.
• Document patient changes and maintain accurate records.
