Qualifications

• Must be a Certified Nursing Assistant (CNA).
• CPR certification required.
• Experience in community health, hospice, or medical/surgical settings preferred.
• Strong communication and patient care skills.

Responsibilities

• Perform personal care and attend to patient requests promptly under supervision.
• Provide compassionate communication to patients and families while maintaining confidentiality.
• Assist with activities of daily living and basic medical tasks as directed by the Case Manager.
• Monitor and report patient condition changes to the Case Manager.
• Assist with meals, mobility, transfers, and basic comfort measures.
• Maintain clean, safe living areas and proper documentation per procedures.
