Job summary

CommuniCare Health Services in Blue Ash, OH is seeking a Hospice Chaplain to provide spiritual care to patients and families within our hospice program. This full-time role supports a compassionate, patient-centered approach in southeast Ohio.

Responsibilities include direct spiritual support, coordination with clergy and care teams, bereavement follow-up, and keeping accurate patient documentation. A seminary education and state licensure/certification are preferred.

Qualifications

• Graduate of an accredited seminary or school of theology is preferred.
• Experience with patients and families facing life-threatening illness or death is preferred.
• Licensure, certification, and/or coursework as required by the state.

Responsibilities

• Providing direct spiritual support to patients and families.
• Working with team members, clergy, and community groups to address spiritual concerns.
• Participating in patient conferences/meetings to assess spiritual needs.
• Providing bereavement follow-up services as indicated by the plan of care.
• Developing and maintaining a resource group of clergy for delegated spiritual care.
• Preparing accurate patient documentation and submitting it.
