VitalCaring Group in the United States seeks a Patient Services Coordinator (PSC) to ensure coordinated care for hospice patients and families. You will connect clinicians, patients, and schedules to support compassionate end-of-life care.

Ideal candidates are detail-oriented, organized, and able to manage multiple priorities in a fast-paced healthcare environment, with emphasis on timely visits and quality communication.

Qualifications

• High school diploma or equivalent required.
• 1+ year of administrative, scheduling, or office experience (healthcare or hospice preferred).
• Strong organizational and multitasking skills.
• Experience with EMR or scheduling systems preferred.

Responsibilities

• Schedule and coordinate patient visits across hospice clinicians to ensure timely, efficient care.
• Collaborate with nurses, social workers, chaplains, and leadership to support daily care coordination.
• Monitor schedules and follow up on missed, incomplete, or delayed visits.
• Utilize EMR and reporting tools to track productivity, coverage, and scheduling optimization.
• Partner with field teams and leadership to reduce missed visits and improve patient experience.
• Communicate effectively with patients, families, clinicians, and referral partners.
• Respond to changing patient needs and scheduling challenges with urgency and problem-solving.
• Maintain accurate documentation and workflow completion within the EMR system.
• Support quality initiatives, including participation in QAPI programs.
