VitalCaring is seeking a Patient Services Coordinator (PSC) in Hospice to ensure timely, coordinated care for patients and families. The role connects clinicians, patients, and schedules in a fast-paced environment with a focus on compassion and professionalism.

You will coordinate visits, collaborate with nurses and social workers, monitor schedules, and utilize EMR tools to maintain accurate documentation and support quality initiatives.

Qualifikationen

• High school diploma or equivalent required.
• 1+ year of administrative, scheduling, or office experience (healthcare or hospice preferred).
• Strong organizational and multitasking skills.
• Experience with EMR or scheduling systems preferred.
• Excellent communication and interpersonal skills.

Aufgaben

• Coordinate patient visits across hospice clinicians to ensure timely care.
• Collaborate with nurses, social workers, chaplains and leadership to support daily care coordination.
• Monitor schedules and follow up on missed or delayed visits.
• Utilize EMR and reporting tools to track productivity and scheduling optimization.
• Promote compassionate, professional interaction with patients and families.
