Job summary

VitalCaring seeks a Patient Services Coordinator (PSC) in Hospice to orchestrate scheduling, patient visits, and care coordination. You will connect clinicians, patients, and families to ensure timely, compassionate end-of-life care.

Ideal candidates will have at least 1 year in administrative or scheduling roles, strong organizational skills, and experience with EMR systems. This full-time role emphasizes empathy, reliability, and clear communication.

Qualifications

• High school diploma or equivalent.
• 1+ year of administrative, scheduling, or office experience (healthcare or hospice preferred).
• Strong organizational and multitasking skills.
• Experience with electronic medical records (EMR) or scheduling systems preferred.
• Excellent communication and interpersonal skills.
• Ability to prioritize, adapt, and problem-solve in real time.

Responsibilities

• Schedule and coordinate patient visits across hospice clinicians to ensure timely, efficient care.
• Collaborate with nurses, social workers, chaplains, and leadership to support daily care coordination.
• Monitor schedules and follow up on missed, incomplete, or delayed visits.
• Utilize EMR and reporting tools to track productivity, coverage, and scheduling optimization.
• Partner with field teams and leadership to reduce missed visits and improve patient experience.
• Communicate effectively with patients, families, clinicians, and referral partners.
