The ideal candidate has a CAPTE-accredited Master’s in Physical Therapy, active state licensure, CPR certification, and a willingness to travel to patient locations within the service area.

Qualifikationen

• Master’s degree in physical therapy (CAPTE-accredited).
• Licensed to practice as a physical therapist in the state of operation.
• CPR certification required.
• Must possess and maintain a valid driver’s license and willingness to travel to patient homes.

Aufgaben

• Provides patient care and documents treatment per physician orders, agency policy, and patient needs.
• Performs patient evaluations and selects appropriate diagnostic material to assess patient care.
• Informs the physician of assessment results and recommended clinical interventions prior to initiating care.
• Develops and documents appropriate treatment plans, selecting intervention approaches and establishing priorities.
• Provides organized treatment including advising, educating and counseling the patient.
• Presents pertinent patient information to physicians and keeps the team informed of status and plan.
• Provides appropriate treatment within agency and payer guidelines.
• Determines measurable goals in collaboration with team and patients.
• Meets productivity standards and uses work hours efficiently.
