Food Management Search, Inc. is seeking a dedicated Physical Therapist for a full-time home health role in the Denver area.

You will evaluate patients, develop and document treatment plans, and provide care within physician orders and agency policies in patients' homes, aiming to improve mobility and comfort. Ideal candidates have at least nine months of PT experience, a CAPTE-accredited degree, active state licensure, CPR certification, and a valid driver’s license to travel to client sites.

Formación

• Minimum nine months experience as a physical therapist.
• Licensed to practice in the state of operation.
• CPR certification required.
• Must possess and maintain valid driver’s license for travel to all business locations.

Responsabilidades

• Provides patient care and documents treatment per physician orders and agency policy.
• Performs patient evaluations and selects appropriate diagnostic material.
• Informs physicians of assessment results and recommended interventions.
• Develops and documents treatment plans, prioritizing interventions.
• Delivers organized patient treatment, including education and counseling.
• Presents patient information to physicians and updates the team on status and plan.
• Provides care within agency and payer parameters and sets measurable goals.
• Meets productivity standards and uses work hours efficiently.

Physical Therapist- Full-time Denver

Are you a dedicated Physical Therapist looking to make a real difference in the comfort of patients' homes? We're seeking a compassionate and skilled PT to join our home health team in the vibrant city of Lancaster. This full-time position offers the unique opportunity to provide personalized care to our diverse client base, helping them achieve their health and mobility goals.

What You’ll Do:

• Provides patient care and documents treatment in accordance with physician orders, agency policy and patient needs.
• Performs patient evaluations in accordance with agency policy and procedure. Selects appropriate diagnostic material to assess all aspects of patient care.
• Informs the physician of the results of the assessment and recommended clinical interventions prior to initiating care.
• Develops and documents appropriate treatment plans for patients including selecting appropriate intervention approaches, strategies, materials and establishing treatment priorities.
• Provides an organized and efficient patient treatment which includes advising, educating and counseling the patient.
• Presents pertinent patient information to physicians and keeps team members informed of patient status and treatment plan.
• Provides appropriate treatment to patients according to patient needs and within the parameters set by the agency and payer.
• Determines appropriate goals, which are objective and measurable in nature, in collaboration with team members and patients.
• Meets agency productivity standards and uses work hours efficiently.

What We’re Looking For:

• Minimum nine months experience as a physical therapist
• Preferred community/home health experience

Required Certifications and Licensures:

• • Master’s degree in physical therapy approved by CAPTE accredited physical therapist education program
  • Currently licensed to practice as a physical therapist within the state of agency operation and compliant with accepted professional standards and practices
  • Must possess and maintain valid CPR certification while employed in a clinical role
  • Must be a licensed driver who can travel to all business locations
