Job summary

Hire Consulting Services is seeking a dedicated Physical Therapist for a full-time home health role in Lancaster. You will provide patient-centered therapy, document treatments, and collaborate with physicians to meet mobility goals.

Applicants should have a master's in physical therapy, current state license, CPR certification, and a valid driver’s license to travel between patient homes. Minimum nine months of experience is preferred.

Qualifications

• Master’s degree in physical therapy from a CAPTE-accredited program.
• Current license to practice in the state of operation.
• Valid CPR certification.
• Must have a valid driver’s license for travel to patient homes.

Responsibilities

• Provides patient care and documents treatment in accordance with physician orders and policy.
• Performs patient evaluations in line with policy and selects diagnostic materials.
• Notifies physicians of assessment results and recommended interventions.
• Develops and documents treatment plans and establishes priorities.
• Educates and counsels patients as part of organized treatment.
• Shares pertinent patient information with physicians and team members.
• Delivers therapy within agency and payer parameters to meet patient needs.
• Sets objective, measurable goals in collaboration with patients and team.
