Essenti Talent is seeking a dedicated Physical Therapist to join our home health team in Lancaster. This full-time position offers the opportunity to provide personalized care to our diverse client base, helping them achieve their health and mobility goals.

The role involves patient evaluations, developing treatment plans, educating patients, and collaborating with physicians to ensure high-quality care while meeting productivity standards.

Qualifications

• Minimum nine months experience as a physical therapist.
• Preferred community/home health experience.

Responsabilités

• Provides patient care and documents treatment in accordance with physician orders, agency policy and patient needs.
• Performs patient evaluations in accordance with agency policy and procedure.
• Informs the physician of the results of the assessment and recommended clinical interventions prior to initiating care.
• Develops and documents appropriate treatment plans for patients including selecting appropriate intervention approaches, strategies, materials and establishing treatment priorities.
• Provides an organized and efficient patient treatment which includes advising, educating and counseling the patient.
• Presents pertinent patient information to physicians and keeps team members informed of patient status and treatment plan.
• Provides appropriate treatment to patients according to patient needs and within the parameters set by the agency and payer.
• Determines appropriate goals, which are objective and measurable in nature, in collaboration with team members and patients.
• Meets agency productivity standards and uses work hours efficiently.
