You will perform assessments, develop and monitor plans of care, document OASIS when allowed, and collaborate with the care team to achieve functional goals.

Qualificações

• Degree in Physical Therapy from an accredited program.
• Current, unrestricted PT license in the state of practice.
• Valid CPR and driver's license.

Responsabilidades

• Perform initial and ongoing assessments of mobility, coordination, strength and function.
• Develop and monitor plan of care; document outcomes and goals.
• Provide written evaluations and discharge plans; deliver PT services as prescribed.
• Coordinate with healthcare team; educate patient and caregivers on home exercise.
• Complete OASIS assessments where allowed by state practice; maintain documentation.
