Job summary

Aveanna Healthcare is seeking a Physical Therapist (PT) to join our home health care team in the Orlando area. You will assess patients, deliver therapy at home, and develop a care plan to promote recovery and independence.

The role requires a PT degree, active license, CPR and driving in order to provide compassionate care in patients' homes. This is a full-time position with weekday visits and ongoing professional collaboration.

Qualifications

• Requires Degree in Physical Therapy.
• Current, unrestricted Physical Therapist license in the state of practice.
• Valid CPR, Driver's License, and proof of automobile insurance.

Responsibilities

• Perform initial and ongoing assessments of mobility, coordination, strength, and function.
• Deliver PT services per physician orders and maintain documentation.
• Develop and implement plan of care with goals, progress notes, and discharge plans.
• Coordinate with healthcare team and family; educate on home exercise program.
• Complete OASIS assessments where state practice allows; supervise PTA/HHA as applicable.
