Job summary

Aveanna Healthcare is expanding its home health PT program in Kent County, Delaware, and seeks a Physical Therapist to join our patients’ care team. This role focuses on evaluating mobility, designing goals, and delivering therapy in patients' homes on a weekly, part-time schedule.

You will perform initial assessments, document progress, collaborate with physicians and caregivers, and support discharge planning.

Qualifications

• Must have degree in Physical Therapy from an accredited program.
• Current, unrestricted state license as a Physical Therapist.
• Valid CPR certification.

Responsibilities

• Perform initial and ongoing assessments of mobility, coordination, strength and current level of function.
• Develop and deliver a written evaluation and plan of care defining PT needs, goals, progress and outcomes.
• Deliver PT services per physician orders and maintain plan of care with timely documentation.
• Coordinate services with healthcare team and family; educate patient and caretakers on home exercise plans.
• Complete OASIS assessments where allowed by state practice and regulation.
