Job summary

Adoration Health is seeking a dedicated Home Health Physical Therapist to join our Albany, KY team. You will provide expert, patient-centered care in a home health setting, with the ability to work independently while supported by a strong team.

Qualifications

• Graduate of CAPTE‑approved PT program.
• Current PT licensure in the state; CPR certification required (may be offered upon hire).
• Two years of adult physical therapy experience; home health experience preferred.

Responsibilities

• Perform physical therapy evaluation and treatment per physician orders, including adaptive equipment.
• Establish measurable goals and develop treatment plans to move patients toward independence and function.
• Maintain communication among treatment team, physician, patient, and family members as applicable.
• Maintain thorough and timely documentation of services for proper billing.
• Supervise PT Assistants and PT Aides in direct patient care per state practice act.
