Adoration Health is seeking a Home Health Physical Therapist to join our team in Albany, KY. You will provide expert, patient-centered care and work 1:1 with patients in Clinton, Cumberland and Monroe counties.

The role offers full-time hours with a 10k sign-on bonus. You will evaluate and treat patients, develop goals, coordinate with the care team, and document services for accurate billing.

Qualifications

• Graduate of a Physical Therapist Education program approved by CAPTE.
• Current Physical Therapist licensure in state of practice; CPR certification preferred.
• Two years of experience in adult physical therapy; home health experience preferred.

Responsabilités

• Perform PT evaluation and treatment based on physician orders and adaptive equipment needs.
• Set measurable goals and develop treatment plans to maximize independence and function.
• Maintain communication with the treatment team, physician, patient, and family.
• Maintain thorough, timely documentation for proper billing.
• Supervise assistants and aides in direct patient care as allowed by state act.
