Job summary

Adoration Health in Ashland, KY is seeking a Home Health Physical Therapist to join our team. You will provide patient-centered PT services in a home setting, Mon-Fri, 8:00 a.m. – 5:00 p.m., with a $15k sign-on bonus for external candidates.

Responsibilities include one-on-one evaluations, goal-driven care, documentation for billing, and collaboration with a multidisciplinary team to support patient independence and family involvement.

Qualifications

• Graduate of a CAPTE-approved Physical Therapist program.
• Current state licensure as a Physical Therapist.
• American Heart Association CPR certification required.
• Two years of adult physical therapy experience; home health preferred.

Responsibilities

• Perform PT evaluation and treatment per physician orders.
• Establish measurable goals and develop treatment plans to improve independence.
• Maintain communication among the treatment team, patient, and family.
• Maintain thorough and timely documentation for proper billing.
• Supervise PTAs and PT Aides in patient care per state act.
