Choice Health at Home is hiring a Physical Therapist to deliver one-on-one home health care, evaluate patients, and design personalized treatment plans. You will guide patients to restore function and mobility, empowering independence in their own homes.

The role requires a CO license, CPR, and a driver’s license with insurance. This full-time position offers benefits and a sign-on bonus, with opportunities to collaborate with an interdisciplinary team.

Qualifications

• Graduate of an accredited Physical Therapy program.
• Current Physical Therapist license in the state of practice.
• CPR certification required.
• Valid driver's license and auto insurance.
• Home Health experience preferred.
• HomeCare HomeBase experience preferred.

Responsibilities

• Perform comprehensive patient evaluations and develop individualized treatment plans.
• Implement therapeutic interventions to restore function, improve mobility, and reduce pain.
• Monitor patient progress and modify treatment plans as needed to achieve optimal outcomes.
• Collaborate with physicians, nurses, caregivers, and interdisciplinary team members.
• Educate patients and families on treatment goals and home exercise programs.
• Maintain timely, accurate documentation and comply with agency policies and regulatory requirements.
• Follow infection control and patient safety standards.

Licensed Physical Therapist in CO
