Job summary

Adventist HealthCare Home Assistance seeks an experienced Certified Nursing Assistant for our Home Care Division in Silver Spring, MD. The role focuses on compassionate, patient-centered care under the Aide Plan of Care, with 40-hour/week schedules and rotating shifts.

Responsibilities include assisting care managers, maintaining safety, documenting care activities, coordinating with the medical team, and supporting discharge planning to ensure continuity of care across shifts.

Qualifications

• Current Maryland Board of Nursing Certified Nursing Assistant (CNA).
• Active American Heart Association Basic Life Support (BLS) certification required.
• Active American Heart Association CPR certification required.
• 2-3 years of CNA experience strongly preferred.
• Nursing home or acute care setting strongly preferred.
• Score of 80% or more on the CNA Skills Assessment Test and ability to pass skills lab testing.

Responsibilities

• Assist care manager with rising patient care needs.
• Provide excellent personal care to patients according to the Aide Plan of Care.
• Maintain a safe environment and address safety and risk management issues.
• Ensure continuity of care across shifts and participate in discharge planning.
• Collaborate with medical team and support staff to deliver quality care.
• Complete patient care duties as assigned.
• Document information on patient records, hospital forms and logs.
• Provide necessary instruction to patient.
• Maintain competencies, license and certifications.
• Demonstrate skills to provide care appropriate to patients’ ages.
• Participate in supply management and equipment maintenance.
• Consult with professional staff on patient problems and goals.
