Job summary

The ideal candidate has a high school diploma or GED, CNA certification in Maryland, BLS, and a valid driver's license.

Qualifications

• High School Diploma or GED required.
• 1 year HHA/CNA experience preferred.
• Maryland CNA certification and BLS required.
• Valid driver’s license with agency insurance requirements.

Responsibilities

• Assists with rising patient care needs.
• Provides personal care to patients per Plan of Care.
• Maintains safe home environment.
• Manages visiting caseload productivity within the expected time frame.
• Documents care in EMR to meet regulatory requirements.
• Consults with professional staff on patient problems and participates in long and short-term goals.
