Job summary

Energy Employee Services, LLC is seeking a compassionate Patient Care Assistant (PCA) to join our home health team. You will help manage the home care of terminally ill or elderly patients, with emphasis on compassionate, accessible, comprehensive care and strong communication with families.

The role requires following the Plan of Care, assisting with hygiene, meals and light housekeeping, and collaborating with the care team to ensure optimal patient outcomes while maintaining timely

Qualifications

• Must be willing to submit to a background check.
• Must be willing to obtain a TB Skin Test if you have not had one in the last year.
• Must be willing to travel to patient’s home (up to a 45 mile radius).
• Must have a valid driver license, auto insurance and reliable transportation.

Responsibilities

• Follow the patient’s Plan of Care (POC) as instructed by the Case Manager.
• Assist patients with hygiene care (including restrooming, transfers, bath and showers) as ordered in the plan of care, to promote quality of life.
• Assist with meal preparation, and light housekeeping (as outlined) to ensure quality of life and best patient care.
• Report all changes of status directly to the Case Manager to ensure best patient care.
• Timely documentation of patient status and condition.
• Must be willing to collaborate with the patient’s care team to ensure best patient outcomes.
• Other tasks as assigned.
