Job summary

The PCA will follow the Plan of Care, assist with hygiene, meals, and light housekeeping, and collaborate with the care team to ensure optimal patient outcomes.

Qualifications

• Background check required.
• TB Skin Test required if not done in the last year.
• Willingness to travel to the patients home within a 45 mile radius.
• Valid drivers license, auto insurance and reliable transportation.

Responsibilities

• Follow the patients Plan of Care (POC) as instructed by the Case Manager.
• Assist patients with hygiene care (including restrooming, transfers, bath and showers) as ordered in the plan of care, to promote quality of life.
• Assist with meal preparation, and light housekeeping (as outlined) to ensure quality of life and best patient care.
• Report all changes of status directly to the Case Manager to ensure best patient care.
• Timely documentation of patient status and condition.
• Must be willing to collaborate with the patients care team to ensure best patient outcomes.
• Other tasks as assigned

As a member of the home health team, you will be responsible for managing the home care of terminally ill or elderly patients. The ideal candidate will also have strong communication skills and a committed dedication to providing patients and families with accessible, comprehensive care.

• Must be willing to submit to a background check.
• Must be willing to obtain a TB Skin Test, if you have not had one in the last year.
• Must be willing to travel to patient's home (This can be up to a 45 mile radius)
