Job summary

Energy Employee Services, LLC is seeking a compassionate Patient Care Assistant to join our home health team in Colerain, OH. You will support terminally ill or elderly patients at home, following the Plan of Care and coordinating with the Case Manager.

The role emphasizes strong communication, reliable transportation, and willingness to travel within a 45-mile radius. A TB test and background check are required, along with a valid driver license and auto insurance.

Qualifications

• Must have a valid driver license, auto insurance and reliable transportation.

Responsibilities

• Follow the patient’s Plan of Care (POC) as instructed by the Case Manager.
• Assist patients with hygiene care (including restrooming, transfers, bath and showers) as ordered in the plan of care, to promote quality of life.
• Assist with meal preparation, and light housekeeping (as outlined) to ensure quality of life and best patient care.
• Report all changes of status directly to the Case Manager to ensure best patient care.
• Timely documentation of patient status and condition.
• Must be willing to collaborate with the patient’s care team to ensure best patient outcomes.
• Other tasks as assigned

We are seeking a compassionate Patient Care Assistant (PCA) to join our team. As a member of the home health team, you will be responsible for managing the home care of terminally ill or elderly patients. The ideal candidate will also have strong communication skills and a committed dedication to providing patients and families with accessible, comprehensive care.

Responsibilities

• Follow the patient’s Plan of Care (POC) as instructed by the Case Manager.
• Assist patients with hygiene care (including restrooming, transfers, bath and showers) as ordered in the plan of care, to promote quality of life.
