CareMed Health Solutions seeks a Nurse Practitioner to deliver primary and preventive care to residents in long-term care facilities in Ohio. You will manage acute changes, perform advanced care planning, and coordinate treatments with an interdisciplinary team.
The role requires OH NP license, board certification, and DEA licensure, with experience in geriatric or adult care and comfort with EMR systems.
Competenze
- Current, unrestricted NP license in OH.
- Board Certification.
- DEA licensure.
- Clinical background in adult, family or geriatric specialties.
- Experience working in a long-term care setting.
- Ability to work with an EMR.
- Ability to manage patients with advanced illness and multiple chronic conditions.
- Ability to work within an interdisciplinary care team to coordinate patient care.
- Psych experience a plus.
Mansioni
- Provide primary and preventative care for long term care patients.
- Manage residents with acute change in condition.
- Advance care planning and chronic care management.
- Prescribe medications, including controlled substances per OH law.
- Order, review and follow up with treatments and diagnostic testing.
- Communicate with nursing home staff about changes in condition.
- Collaborate with primary care physician on care plan.
