Job summary

Wheeler Mission Ministries is seeking a Case Manager to provide a continuum of care for guests through assessment, plan development, and goal attainment. You will work with guests to identify personal goals and create plans to achieve stability and growth within a faith‑based service context.

The role emphasizes collaboration with program staff, maintenance of confidentiality, and participation in multidisciplinary case conferences while supporting guests on their path to stability.

Qualifications

• High School Diploma or equivalent.
• 1–2 years of experience in social services or ministry, specifically counseling, mentoring, and discipling.
• At least 1 year of experience working with individuals who may have experienced substance abuse, mental health concerns, trauma, domestic violence, or homelessness.

Responsibilities

• Assess and identify guests’ resources, talents, skills, and interests.
• Develop a care plan with written goals and methods to help the guest achieve those goals.
• Monitor the plan’s effectiveness and modify as needed.
• Advise guests on best practices for success and advocate for them.
• Identify needs and resources, provide information, referrals, and lead crisis interventions when necessary.
• Monitor guest progress and ensure program confidentiality.
