Job summary

Ohana is hiring an Extended Foster Care Success Coach in Tucson, AZ to support young adults exiting foster care. The role focuses on building trusting relationships, helping set goals, securing stable housing, and guiding participants toward employment.

You will collaborate with Ohana and community services, maintain case notes in Guardian, participate in monthly trainings, manage a caseload up to 20, and respond to crises with on-call support.

Qualifications

• Bachelor’s or associate degree in an appropriate discipline.
• At least 21 years of age.
• Valid driver’s license with current car insurance.

Responsibilities

• Carry a caseload of up to 20 young persons.
• Build supportive relationships and help set achievable goals.
• Coordinate with supervisors and community resources to support youth.
• Conduct monthly in-person contacts and monthly collateral contacts.
• Respond to non-emergency outreach and be part of on-call emergency coverage.
• Support housing stability and employment readiness for participants.
