Job summary

Ohana AZ is launching an Extended Foster Care program in Flagstaff, seeking full-time Success Coaches to guide young adults through transition. You will work with young adults, mentors, and community partners to promote goals, housing stability, and employment readiness.

The role requires a bachelor’s or associate degree or relevant experience, a valid driver's license, and a commitment to trauma-informed, healing-centered practices. Training and ongoing development are provided.

Qualifications

• Knowledge of trauma-informed care and healing-centered approaches.
• Shall possess a bachelor’s or associate degree in appropriate discipline or related experience.
• Ability to maintain a caseload and coordinate with multiple services.
• Strong communication and motivational interviewing skills.
• Ability to build supportive relationships with young adults and mentors.

Responsibilities

• Carry a caseload of up to 20 young persons.
• Maintain supportive relationships with young adults and mentors.
• Conduct monthly supervisory clinical sessions.
• Perform weekly check-ins with supervisor and report needs.
• Coordinate with state services and community resources to support goals.
• Lead Transition Plan Staffing with the team.
• Assist young adults before 18 with voluntary agreements as needed.
• Complete in-person and collateral contacts monthly.
• Respond to outreach and participate in rotating on-call duty.
• Maintain Guardian notes within required timelines.
• Support EFC reviews and participate in service planning.
• Provide transportation when required to meet goals.
• Complete required trainings and background checks.
