• Provide supportive services to nursing staff and perform assigned duties.
• Assist residents with activities of daily living per care plans and policies.
• Assist with transfer, positioning, and mobility of residents.
• Coordinate dining room service, meals, and documentation of intake.
• Deliver nutritional supplements and document intake as required.
• Record weights and report changes to nurse/supervisor.
• Monitor skin condition and report any issues to nurse/supervisor.
• Assist with toileting program activities as directed.
• Complete flow sheets and document resident progress.
