Job summary

Central Florida Behavioral Hospital in Orlando, FL, seeks a Discharge Planner to ensure smooth, safe transitions from inpatient behavioral health treatment back into the community, bridging clinical care and aftercare to maintain continuity and reduce readmission risk.

You will arrange post-discharge services, meet with patients to review plans, confirm medications and refills, and involve families in planning with clear, patient-friendly language.

Qualifications

• Bachelor's Degree in social work or related field required.
• Two (2) years' experience working in a psychiatric setting preferred.
• Handle with Care, Verbal Deescalation and BLS/CPR required (May be obtained upon hire).
• Effective Oral and Written Communication Skills
• Must be available to attend the full hospital and departmental orientations.

Responsibilities

• Arrange and follow up on post-discharge services such as therapy, medication management, housing, and community support
• Meet with patients to review the discharge plan and ensure patient understanding
• Ensuring patients leave with prescribed medications and refills, and understanding how to access them
• Involving patients and their families in planning, providing education, and addressing questions or concerns in understandable language
