Heart to Heart Hospice in Oklahoma City seeks a compassionate Hospice Aide (CNA) to provide direct patient care under the supervision of an RN in Oklahoma City. You will follow the Plan of Care and assist with transfers and mobility as needed.

Required CNA license, high school diploma or GED, and at least one year in hospice or nursing assistance; EMR experience is a plus. A valid driver’s license and reliable transportation are also required.

Formación

• High School Diploma or GED preferred.
• Unencumbered CNA license required.
• At least 1 year experience as Hospice Aide or Nursing Assistant.
• Demonstrates compassionate care and maintains confidentiality.
• EMR experience is a plus.
• Valid driver's license and reliable transportation with ongoing auto liability insurance.

Responsabilidades

• Provide direct patient care under RN direction and according to Plan of Care.
• Assist patients with safe transfers and ambulation per policy.
• Document care provided and report changes to ensure continuity of care.
• Practice infection control principles.
• Promote respectful communication with patient, family, and staff.
• Timely submission of all documentation.
• Perform other duties as required.
• Ensure compliance with laws, Medicare regulations and policies.
