Job summary

Heart to Heart Hospice in Texas is seeking a dedicated Hospice Aide (CNA) to provide direct patient care under the supervision of an RN. You will support safe transfers, assist with daily activities, and document care to ensure continuity.

The ideal candidate holds an unencumbered CNA license, a high school diploma or GED, and at least one year of experience in hospice or long-term care. A valid driver's license and reliable transportation are required.

Qualifications

• High School Diploma preferred or GED.
• Must have an unencumbered CNA license.
• At least one year experience as Hospice Aide or Nursing Assistant.
• EMR experience a plus.
• Must have a valid driver’s license and reliable transportation.
• Maintain confidentiality and integrity of patient information.

Responsibilities

• Provide direct patient care under the RN and Plan of Care.
• Assist with safe transfers and ambulation per policy.
• Report changes and document care to ensure continuity.
• Practice infection control principles.
• Communicate respectfully with patient, family and staff.
• Timely submission of documentation.
• Ensure compliance with laws, Medicare regulations and policies.
