Job summary

Advocate Health in Wisconsin seeks a Home Health Aide to cover Washington, Ozaukee and Dodge counties. Work Monday–Friday, 8:00–16:30, 40 hours weekly, providing compassionate patient care in patients' homes.

Responsibilities include assisting with ADLs, monitoring vital signs, documenting care per agency standards, and collaborating with the care team to support hospice goals and patient safety.

Qualifications

• Written and verbal English competency.
• Must be certified as a nursing assistant in the state.
• Valid driver’s license required.

Responsibilities

• Contributes to effective patient outcomes and provides competent home health aide care.
• Assist with ADLs to help patients reach functional goals.
• Notice changes in patient condition and inform supervisor with documentation.
• Provide care consistent with hospice philosophy and safety.
• Perform housekeeping tasks and monitor patient safety.
• Record vital signs as directed and maintain patient comfort.
• Ensure skin care and repositioning to prevent breakdown.
• Maintain cleanliness and appropriate linens.
