As a Home Health Aide, you will provide personal care and assistance to patients in their homes, under the supervision of nursing staff. Responsibilities include daily living support, care plan review, and home maintenance tasks.

Responsibilities

Job Summary

As part of our interdisciplinary Hospice team, our CNAs (Home Health Aides) provide patients with care in the comfort of their own home and/or similar setting.

As a Home Health Aide in Hospice you will provide direct assistance with activities of personal care, maintenance of patient health, agency facilitation of treatment for patients, and contribution to a healthy environment for patients. These activities are provided under the direction and supervision of a Registered Nurse or Therapist, in accordance with the physicians' orders.

Responsibilities

• Support patient care by reviewing the care plan in the home prior to providing care. Notify the Case Manager with issues or concerns regarding the care plan and when care needs revision.
• Provide assistance with daily living under the supervision of the Case Manager, e.g., bathing, dressing, transfers, range of motion, toileting, etc.
• Explain to the patient the care being provided and involve the patient in care.
• Consistently complete mandatory care upon arrival and departure from patients' homes in accordance with agency policy. Mobile Care includes tasks completed in accordance with the care plan. Complete mileage logs timely and accurately.
• Work cooperatively with patients, care providers, family members, and appropriate agency staff by listening and responding to patient needs, engaging in appropriate communication, and reporting pertinent patient information to the Case Manager in a timely manner.
• Regularly complete home maintenance per plan of care, e.g., house cleaning, washing dishes, laundry, meal preparation, and transportation, etc.

Qualifications

Schedule & Location

Full‑time, 1st shift – Monday‑Friday 8:00 a.m.–4:30 p.m.
