Responsibilities

• Provide routine daily nursing care and services per care plan.
• Identify residents before procedures or meals using wristband/photo ID.
• Report changes in residents' condition to Nurse Supervisor/Charge Nurse promptly.
• Record entries accurately on flow sheets, notes, and charts.
• Ensure resident room is prepared for admission (bed, tags, supplies).
• Assist with meals, personal care, and daily activities as directed.
• Maintain confidentiality of resident information and report any disclosure violations.
• Observe and report pressure areas and skin integrity to prevent ulcers.
• Assist with transfers, lifting, turning, and positioning of residents.
• Participate in nursing shift reports and communicate effectively with team.
