Qualifications

Responsibilities

• Identify residents before treatments or meals using wristbands or photo cards.
• Record entries accurately and maintain confidentiality of resident information.
• Report changes in resident condition to Nurse Supervisor/Charge Nurse promptly.
• Assist residents with daily activities and personal care as directed by care plans.
• Ensure rooms and equipment are clean, organized, and ready for residents.
• Support load/unload of residents and assist with transports as needed.
