Job summary

Sage Creek Post-Acute is seeking a compassionate Certified Nursing Assistant to provide routine daily nursing care to residents in accordance with care plans. You will identify residents, assist with daily activities, and document observations for the nursing team.

You will support meal service, mobility, and personal care while maintaining a respectful, safe, and comfortable environment. CPR certification preferred; a licensed CNA is required by state regulations.

Qualifications

• Must be a licensed Certified Nursing Assistant in accordance with state laws.
• CPR certification preferred.
• Ability to read, analyze, and interpret general information.
• Ability to write reports and communicate effectively.

Responsibilities

• Identify residents using wristband or photo card before care.
• Record entries on flow sheets, notes, and charts in a descriptive manner.
• Report changes in resident condition to Nurse Supervisor promptly.
• Maintain confidentiality of resident information including HIPAA.
