Job summary

Qualifications

• Must possess, as a minimum, a 10th grade education.

Responsibilities

• Identify residents before administering treatments using wristband/photo card.
• Record entries clearly on flow sheets, notes, or charts.
• Report changes in residents' condition to the Nurse Supervisor/Charge Nurse.
• Ensure resident rooms are ready and greet/escort residents to their rooms.
• Assist residents with daily living activities and personal care.
• Maintain confidentiality of resident information and report breaches.
• Follow policies on exposure to blood and body fluids.
• Assist with moving/transporting residents between rooms and to vehicles.
• Observe and report skin issues to prevent pressure ulcers.
• Participate in nursing report and follow established procedures.
