Job summary

Qualifications

Responsibilities

• Review and implement ISP and BSP.
• Provide quality, person-centered support.
• Monitor health and well-being of individuals.
• Follow CSS policies and procedures for care.
• Submit required documentation by deadlines.
• Maintain open communication with supervisors and stakeholders.
• Complete DSP scheduling and on-call responsibilities.
• Transport individuals during scheduled hours.
