Physicians’ Primary Care of Southwest Florida is seeking a Care Coordinator for our Family Practice Division in Fort Myers, FL. This full-time role offers a Mon-Fri daytime schedule in our Administration building, working closely with medical staff to coordinate chronic disease management and patient outreach.

Qualified candidates have at least 3 years of clinical experience and hold a Florida PN or RN license. Excellent attention to detail and HIPAA compliance are essential.

Qualifications

• Three years of clinical experience required.
• Florida PN or RN licensure required.
• Strong attention to detail and accurate record-keeping.
• Ability to coordinate care with providers and staff.

Responsibilities

• Develop and implement chronic disease management programs.
• Review schedules and patient charts to coordinate visits and future healthcare needs.
• Coordinate patient outreach programs.
