Job summary

Little River Medical Center, Inc. in Myrtle Beach, SC seeks a Community Health Worker (CHW) to engage women in the WISEWOMAN program. The CHW will support navigation of health and social services, provide health coaching, and conduct home visits as a frontline public health professional.

The role focuses on low‑income, uninsured or underinsured women (ages 35-64) to reduce cardiovascular risk factors and improve access to care, housing, food, transportation, and mental health resources.

Qualifications

• Strong people and communication skills, both verbally and in writing.
• Self-motivated, able to work independently and with minimal supervision.
• Ability to gather data, collect and compile information, and prepare reports.
• Must have a valid driver’s license and provide own transportation.
• Spanish language ability preferred but not required.

Responsibilities

• Identify women eligible for the WISEWOMAN Program (ages 35-64, low income).
• Assess social needs and facilitate referrals to social services and programs.
• Provide health coaching and assist with attending WISEWOMAN appointments.
• Document all contacts and follow up to support ongoing engagement.
• Build relationships with community partners and navigate health and human services.
