Position: AHH - Community Health Worker (CHW)
Location: Myrtle Beach, SC
Job Id:1233
# of Openings:1
The Community Health Worker (CHW) works in conjunction with the Accesshealth Horry BCN (Best Chance Network) team to engage women in the State’s WISEWOMAN program. The CHW works with low-income, uninsured, or underinsured women (ages 35-64) to lower their risk of heart disease and stroke. The purpose of the program is to help reduce major cardiovascular disease (CVD) risk factors like high blood pressure, high cholesterol, diabetes, and obesity. The WISEWOMAN CHW will work with women in the Pee Dee Region, and other regions in the State, around their social determinants of needs (food, housing, transportation, health care, mental health, etc.) for the purpose of engaging them in the program and lowering their risk factors. The CHW spends 60% of their time out in the community and is considered a “front‑line” public health professional who works with women who need the WISEWOMAN program. The CHW will help individuals navigate both the health and human services system; provide health education and life coaching; help identify gaps in services and provide care coordination follow‑up. The CHW is expected to provide support, encouragement, and guidance to individuals who need the most assistance. The goal of the CHW is to be a positive change‑agent and help engage women in the WISEWOMAN program. Looking for someone who has “lived” experience, loves to get out and help others in need, and is familiar with community resources. Note: Formal education is not a requirement.
