Overview

We are seeking a dedicated Community Health Worker (CHW) to serve patients in the Grandstrand area. A CHW has a strong understanding of the community served and acts as a liaison between health and social services and the community to facilitate access to services and to improve the quality and cultural competence of service delivery. CHWs perform visits in homes and facilities (ALF and ILF) in their designated service area.

• Serve approximately 200 patients and collaborate with the care team and health coaches to provide a variety of services.
